Step 3: Log into your Patient Portal and upload the signed documents. That’s it! Or you can email them to us at drmarzell@drmarzell.com and Drea will enter them into your chart for you.

Please preventively fill out the Records Release with your primary care physician’s information or the last doctor you were working with so that we may obtain your medical records.

Please also take a photo of the front and back of your Insurance cards to upload as well. We call on every new patient to ensure you are covered and notify you of any costs prior to the appointment.

NOTE: If you have not received an email from us inviting you to register for the Patient Portal, please call us and ask us to re-send it to you manually. You may want to check your SPAM folders prior to calling, just in case.
